Eligibility Criteria
To qualify for First Horizon's Structured Family Caregiving (SFC) program, individuals must meet specific eligibility criteria to ensure they receive the right level of support. Our team is here to help you understand these requirements and guide you through each step of the process.

Care recipient:
- Must be an Indiana resident
- Enrolled in the Health and Wellness Waiver or TBI Waiver
- Require assistance with at least 3 personal care tasks per day
- Live with the caregiver

- Must be at least 18 years old
- Physically and mentally capable of caregiving
- Willing to undergo a criminal background check
- Must have no history of abuse, neglect, or exploitation
